In consideration of my participation in the 5K Hope Run (the “Event”), I hereby acknowledge, understand, and agree to the following:

1. Assumption of Risk: I know that participating in a run/walk event is a potentially hazardous activity. I understand that I should not enter or participate unless I am medically able and properly trained. I assume all risks associated with participation, including but not limited to falls, contact with other participants, effects of weather, traffic, and conditions of the course. *

2. Release and Waiver: I, for myself and on behalf of my heirs, executors, administrators, assigns, and next of kin, hereby release and hold harmless Iglesia Faro de Luz, Hogar Faro Kids, event organizers, volunteers, sponsors, the City of Katy, and Mary Jo Peckham Park, and their respective representatives and agents from any and all liability, claims, or causes of action arising out of my participation in the Event. *

3. Medical Authorization: I grant permission to event staff, volunteers, and emergency personnel to provide or arrange for medical assistance, transportation, or treatment in the event of an injury or illness, and I agree to be fully responsible for any costs associated with such services. *

4. Photo & Media Release: I authorize the use of my name, image, and likeness in photographs, videos, or recordings taken during the Event for promotional or fundraising purposes by the organizers, without compensation. *

5. Understanding and Agreement: I have read this waiver and fully understand its terms. I sign it freely and voluntarily and intend it to be a complete and unconditional release of all liability. *
